MyChart Instructions

Please take the time to log in to MyChart. This is a secure way to communicate with us and us with you. You can sign up by clicking the “New User” tab on the link below,  just as it appears in the picture below. If you have not received an activation code from our office or in the mail, click “no activation code” to continue – this will allow you to set up your MyChart using other basic health information. Please contact the office if you need your activation code to finish the set up process.

Once you are enrolled in MyChart you may request refills or referrals, and communicate with the office. We will be able to confirm that your refills or referral have been done at your request. Your basic patient information should be visible to you within the portal links and as we move forward your clinician may discuss sending your lab results through this portal as well.
